Authorization to Give Medicine Form
Virginia State Health Form
This is a required form for campers enrolled in the Great Adventures division (3 year olds/ rising Preschoolers - rising Kindergarteners) and may be downloaded from the link above.
This form MUST be signed and dated by a physician and submitted at least one week prior to the camper’s first session.
Important Notice: Online registration does not automatically enroll your children. You must submit the Congressional Camp Emergency/Health History Form and Proof of ID, if it has not been previously submitted. These documents complete the application which is processed on a first come, first served basis.
